Initial Periodontal Exam

When a new patient calls our office for an appointment, we will send a packet containing information about our practice as well as patient registration and medical history forms. By completing these forms at home before your appointment it allows us the opportunity to spend quality time with you when you arrive. These forms can either be downloaded or printed by clicking here or you may register electronically online  on our secure server.

A comprehensive evaluation is usually necessary to properly evaluate your mouth and make treatment recommendations. Our first meeting will give us an opportunity to become acquainted so that you will learn about us and we will understand your oral health goals. We have reserved this amount of time so you will have the opportunity to openly discuss your concerns with us. During this time we will perform a comprehensive examination and obtain any additional x-rays or molds that may be important in providing a diagnosis and treatment of your condition. Depending upon the complexity of your situation it may be necessary to schedule a separate consultation visit. An interactive discussion of your diagnosis and treatment options will take place at that time. If the necessary care is of an isolated nature it is likely that the exam and consultation can be completed at your first visit.

We work closely as a team with your referring dentist, but a referral by a dentist is not required for a visit to our office. We welcome referrals from patients and friends of the practice.

Note:

Please assist us at the time of your initial visit to the office by providing the following information:

  • Your referral slip and x-rays from your referring dentist. If your referring dentist has taken X-rays, you may request that they be forwarded to our office. We would be happy to assist you in this matter.
  • A list of medications you are currently taking.
  • If you have dental insurance, please bring any forms or insurance cards with you to the first appointment.
